STEP 2: Make copies of both the title page and the copyright information page on back (verso) of the title page of each book or publication information of the magazine. Paperclip materials together with appropriate title designation and credits in order.
STEP 3: Make copies of all the pages to be included in your course pack.
STEP 4: Submit the completed forms to Copyright Manager with copies of all the pages to be copied in your course pack, the title page and copyright information from the back (verso) of the title page.
